Mixed Neuroendocrine-non-neuroendocrine Neoplasm of the Sigmoid Colon

症例は81歳男性で，便通異常を主訴に前医を受診した．下部消化管内視鏡検査でS状結腸に全周性の2型病変を認め，生検では中分化腺癌が検出された．CT検査では明らかなリンパ節転移や遠隔転移は認めず，同病変に対して腹腔鏡下S状結腸切除術を施行した．術後病理組織検査では，中分化腺癌と，synaptophsinが陽性，Ki67陽性細胞が80％以上のNET G3を伴う部分を認め，mixed neuroendocrine-non-neuroendocrine neoplasm（以下MiNEN）と診断した．高齢であり，術後補助療法を施行せず経過観察の方針とし，術後3年現在，再発なく経過中である．MiNENは予後不良な疾患群であるため，多くの症例は術後補助療法が施行されているのが現状である．術後補助療法を施行せず長期無再発生存中の大腸MiNENの報告はなく，文献的考察を加えて報告する．
